Payment Authorization & Financial Policy
A full week of service must be paid in advance before the first day of care.
Weekly payments are due every Monday to secure caregiver scheduling.
Services are billed from Monday through Sunday.
The first week's service is non-refundable once scheduled.
Payments are required to ensure caregiver compensation.
$25 late fee applies if not paid by Monday at 6:00 PM.
Services may pause if not paid within 72 hours of the due date.
NON.REFUNDABLE TERMS
All services must be paid in advance and are non-refundable.
Ambition HomeCare LLC reserves caregiver availability based on prepayment
Missed or canceled visits do not qualify for refunds.
HOSPITALIZATION / EMERGENCY DISRUPTION
Services will pause if the client is hospitalized or an emergency occurs.
Client or representative must notify the agency immediately.
Unused days may be credited only at Ambition HomeCare LLC's discretion.
Refunds are not guaranteed.
CLIENT ACKNOWLEDGMENT
I understand payment is due every Monday and covers services for the week.
I understand caregiver schedules depend on my payment.
I agree that unused care days are non-refundable after service begins.
I consent to the collection, use, storage, and processing of my personal and, where applicable, health-related information, including any data I submit on behalf of others, for the purpose of evaluating or fulfilling my request made through this form. I understand this will be handled in accordance with the Privacy Notice.
